DMCA Guidelines

Welcome to All Vermont Forms, your go-to source for finding and filling out various forms relevant to the beautiful state of Vermont. In our commitment to respecting and protecting intellectual property rights, we adhere to the Digital Millennium Copyright Act (DMCA). This page outlines our DMCA guidelines and explains the process for submitting a takedown notice if you believe your copyrighted work has been posted on our site without permission.

Understanding the DMCA

The DMCA is a U.S. federal law that provides guidelines for online service providers in case of copyright infringement. By complying with these guidelines, All Vermont Forms ensures that copyrighted materials are promptly removed from our site if they are posted without consent.

Submitting a Takedown Notice

If you are a copyright owner or an agent thereof and believe that your copyright has been infringed upon by content hosted on our platform, please notify us. To make a claim, you must provide a written communication that includes the following:

  • Identification of the copyrighted work that you claim has been infringed upon.
  • Identification of the content on our site that you claim is infringing, with enough detail so that we may locate it.
  • Your address, telephone number, and email address.
  • A statement by you that you have a good faith belief that the disputed use is not authorized by the copyright owner, its agent, or the law.
  • A statement by you, made under penalty of perjury, that the above information in your notice is accurate and that you are the copyright owner or authorized to act on the copyright owner's behalf.

Please send your takedown notice to dmca@vermontform.com. We will review your submission and take appropriate action in accordance with the DMCA and our policies.

Counter-Notice

If you believe that your content has been mistakenly removed due to a takedown notice, you may submit a counter-notice. This must include:

  • Your physical or electronic signature.
  • Identification of the content that has been removed or to which access has been disabled and the location at which the content appeared before it was removed.
  • A statement under penalty of perjury that you have a good faith belief that the content was removed or disabled as a result of mistake or misidentification.
  • Your name, address, and telephone number, and a statement that you consent to the jurisdiction of the federal court in Vermont, and that you will accept service of process from the person who provided notification of the alleged infringement.

If a valid counter-notice is received, we will send a copy to the original complainant, informing them that the removed material may be replaced or access to it restored in 10 business days. Unless our complainant files an action seeking a court order against you, the material will be restored in 10 to 14 business days after receiving the counter-notice.
